Presidential Sports Award Certificate

To receive your Fitness Award Certificate, signed by the President of the United States, send the following three items to:

Presidential Sports Award/AAU

P.O Box 10,000
Lake Buena Vista, FL 32830-1,000

1. A Completed copy of  the above Fitness Log
2. A Completed copy of this Request Form
3. A Check for: $8.00 per award for US and APO/FPO delivery
($10.00 per award for Canada, $15.00 per award for all other countries, US Currency)
made payable to: Presidential Sports Award/AAU
